What is the VA Community Care Program?
The VA Community Care Network is the VA’s direct link to community healthcare providers, ensuring that veterans receive timely and comprehensive care outside VA facilities.
- Eligibility: Open to veterans enrolled in or eligible for VA health care.
- Approval Required: Veterans must first obtain authorization from their VA health care team to receive treatment from a community provider.

How to Get Started
To begin receiving care at our practice through the VA Community Care Program, follow these steps:
- Contact Your VA Primary Care Provider: Request a referral to our office, listed in the VA Community Care Directory under “Dentist – General Practice.”
○ Provider Name: Thomas Stack, DDS - Obtain Your Referral: Once your referral is approved, you can either schedule your appointment directly with us or ask your VA health care team to schedule it for you.
